Health Care Directive

There are generally three (3) categories of Advance Health Care directives which specify a person’s wishes regarding medical treatment:

  • Living Will,

  • Power of Attorney and

  • Health Care Proxy (also known as a health care surrogate or durable medical power of attorney). 

Having such a directive in place may be necessary near the end of life or at any time due to an accident, injury or illness so that your family does not need to obtain court orders to handle our medical situation.  Your attorney can provide you with the appropriate document to express your preferences for medical treatment in an extreme medical situation when you cannot communicate. 

When completing these documents, they each typically require an acknowledgment, which is a notarial act that ensures a signer’s identity and willingness to sign.  They usually also require two (2) adult witnesses.  When booking an appointment with Bergen Pro Notary, be sure to specify whether you need witnesses along with your notarization.
